Residential Services Program Coordinator
Position: Residential Services Program Coordinator, in the Community Living Services Department
Schedule: Monday-Friday, 40 hours per week. On-call 24-hours on rotating basis.
Starting Salary Range: *$23.00-$26.95/hour

As a Residential Services Program Coordinator you will coordinate 24-hour residential services for caseload of participants served in Laradon's CLS Department. Train and support contractors. Interact with department director to ensure quality of life and upward development of participants
No day is ever alike but you can generally look forward to the following responsibilities:
• Monitor and implement individual residential programming for participants on caseload.• Ensure participants’ health and safety needs are met.
• Monitor participants’ medical needs.
• Support participants and independent contractors (ICs) with participants’ finances.
• Prepare for and attend Service Plan (SP)/Interdisciplinary Team (IDT) meetings.
• Develop and train ICs to implement participants’ ISSP programs according to identified SP outcomes.
• Monitor Individual Service and Support Plans (ISSPs) tracking monthly and revise programs as needed.
• Monitor participant main and working files and complete follow-up.
• Ensure program adheres to regulatory agency guidelines.
• Review participant incident reports, identify trends and provide necessary follow-up.
• Ensure Critical Incident Reports are completed and filed within designated timelines and ensure follow-up as needed in accordance with regulations.
• Complete and submit Human Rights Committee (HRC) paperwork and follow-up on recommendations according to established deadlines.
• Assist and participate in quality assurance procedures and policy development to ensure compliance with governing agencies.
• Establish and maintain written and verbal communication with governing agencies, families, guardians, ICs and other professionals.
• Maintain benefit eligibility, i.e., Social Security, Medicaid, Medicare, etc.).
• Conduct and document monthly host home visits and provide training and follow-up as needed.
• Facilitate and attend IC functions as needed.
• Participate in monthly residential services management meeting.
• Develop host home placements for participants.
• Maintain IC contract files.
• Ensure ICs are in compliance with contract.
• Assist with budgets, policies, procedures and strategic plans.
• Provide 24-hour crisis intervention and counseling to participants and ICs as needed.
• On-call 24-hours on a rotating basis.
• Maintain regular and punctual attendance.
• Other duties as assigned.

We believe in providing continuing education and development opportunities for our employees. To start you off right in your position, we’ll make sure you are trained in the following:
- All trainings deemed mandatory by Laradon.
- A minimum of 24 hours of approved training annually.
- Orientation,
- First Aid/CPR.
- Medication Certification.
- Crisis Intervention training
- 20 hours of on-the-job training.
We ask that you bring the following assets to our workplace:
Bachelor’s degree in Human Services or related field.
Five years direct experience may be substituted for degree requirements.
Excellent verbal and written communication and problem-solving skills.
Good customer service and people skills.
Valid driving license, good driving record, and reliable personal transportation.
Must be able to lift, push and pull 25 pounds
Mobility, bending, sitting, stooping.
Stamina to be on feet and active for full work day.
